About The Position

Job Summary: The BH Licensed Clinician is responsible for utilizing their subject matter expertise across the continuum of care, to provide mental health services, consultation, care coordination and other targeted interventions to help promote positive health outcomes.

Requirements

  • Master's degree in Social Work or Counseling from an accredited college or university.
  • 3 years of Behavioral Health experience.
  • Must have a current licensure in one of the following: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) from the Texas State Board of Social Work Examiners or the Texas State Board of Examiners of Professional Counselors.
  • Certification in Non-Aggressive Crisis Management; must be obtained while in orientation and maintained throughout employment.
  • Current JPS recognized CPR certification and must be maintained throughout employment. May be obtained subsequent to hiring.

Nice To Haves

  • Master's Degree in Social Work with experience in an acute and outpatient behavioral health care settings.
  • 2 plus years of experience in Population or Disease Management.

Responsibilities

  • Utilizes behavioral health expertise in order to promote change to improve behavioral and physical health conditions and coordinate the care of an identified population throughout the healthcare continuum.
  • Meets with the team, including providers and other clinical/non-clinical staff to contribute to the development of a comprehensive plan of care based on a systematic assessment of the physiological and psychosocial needs of the patient.
  • Evaluates and modifies plans of care as needed. May provide ongoing follow-up between hospitalizations/outpatient visits.
  • Monitors patient progress towards established goals.
  • Coordinates and facilitates communication between members of the care team including providers, clinicians, staff and families.
  • Collaborates with patient, family and healthcare team to arrange for identified home care needs. Acts as a hospital liaison for agencies and individuals involved with selected patients.
  • Performs appropriate screenings/clinical assessments of mental health and substance abuse conditions.
  • Documents all patient care activities appropriately in the medical record.
  • Develops and maintains data system to track patients and outcomes.
  • Identifies quality Improvement initiatives.
  • Maintains timely, effective communications with the department leadership regarding all events or proposed changes which might impact regulatory issues, census variance, policies or procedures.
