Requirements

• Bachelor's in counseling, psychology, social work or another related field. Master's degree in licensable field strongly preferred.
• Must qualify as a MHP or higher per Illinois Department of Mental Health Rule 132.
• Illinois Certified Addiction Counselor (CADC), or licensed in Illinois as a LSW, LPC, or higher, within 2 years of employment.
• Knowledge of DSM diagnoses and appropriate criteria for each diagnosis.
• Knowledge of accepted assessment techniques.
• Ability to quickly establish rapport.
• Computer proficiency required.
• Ability to navigate and complete documentation in the electronic healthcare record.
• Ability to accurately complete required documentation within the prescribed time frames.
• Self-starter with ability to work independently.
• Ability to work as a team member.
• Valid driver's license and ability to meet Agency insurance requirements (designated positions).
• Must have own transportation (designated positions).

Responsibilities

• Work hours prescribed by the leadership and any additional hours deemed necessary for the responsibilities assigned.
• Provide centralized diagnostic assessments that include computerized social histories, mental status, presenting problem, diagnosis, and treatment needs.
• Determine appropriate level of care and treatment using centers utilization management criteria, develop corresponding ISP with clients, and coordinate needs with appropriate staff.
• Identify the need for immediate crisis intervention services.
• Maintain documentation per center quality assurance procedures and clinical performance standards.
• Assess client needs and strengths and establish a comprehensive plan that addresses support and treatment needs necessary for less restrictive care and stabilization to occur safely.
• Provide linkage, referrals and advocacy with community resources and community organizations.
• Cooperate with work team/center/on accomplishment of strategic objectives and activities.
• Ensure that the clinical treatment needs are met for consumers, which will enable them to work towards recovery.
• Educate community agencies regarding services offered by Rosecrance.
• Serve as a member of the Access team and participate in all team meetings and activities.
• Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
• Deliver exceptional customer service consistently to every customer.
• Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
• Assume other related responsibilities as delegated and assigned.
