About The Position

By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families. The Bereavement Practicum Trainee in bereavement works under supervision of a Licensed Marriage and Family Therapist, or Licensed Clinical Social Worker, or Licensed Professional Clinical Counselor, and provides individual and group counseling services to patients/families to address their emotional needs related to the impact of a terminal illness, loss, and grief. By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Requirements

  • Current enrollment and good standing in an accredited Master’s degree program in a mental health discipline including counseling psychology or social work
  • Volunteer and/or work experience in a helping profession including hospice, medical setting, caregiving or crisis intervention preferred

Responsibilities

  • Assessment Intakes to identify psycho-social and grief counseling needs of bereaved individuals, and to determine if needs can appropriately be met by services offered by the Bereavement Department of By the Bay Health.
  • Individual and Group grief counseling for those mourning the loss of a loved one (remote and in-person)
  • Telephone outreach and support to bereaved who have lost a loved one on BTBH hospice service
  • Documentation in an Electronic Medical Record, daily updating of multiple administrative logs and other administrative tasks
