About The Position

Assists the Bereavement Program Coordinator with ensuring bereavement services are provided to the families/caregivers of hospice patients in accordance with the interdisciplinary plan of care. Bereavement counseling consists of counseling given to the individual's family after the individual's death. Bereavement counseling is a required hospice service, provided for a period up to one year following the patient's death. Bereavement counseling will ensure that the bereavement services reflect the needs of the bereaved.

Requirements

  • Master's degree in social work or related field, or equivalent experience.
  • Valid Driver's License and Automobile Insurance.
  • Basic Life Support (BLS) Certification.
  • 2 years of recent relevant experience.
  • Understanding of psychosocial implications of illness and dynamics of grief.
  • Knowledge of Medicare and regulations applicable to Bereavement programs.
  • Excellent computer skills.

Nice To Haves

  • Experience in hospice and/or home care.
  • Knowledge of community resources available to bereaved persons.
  • Excellent organizational and interpersonal skills.
  • Ability to work effectively with minimal supervision.
