About The Position

Bereavement Coordinator: Concordia Hospice of Washington is seeking a compassionate and organized Bereavement Coordinator to support patients’ families and loved ones through the grieving process. This role is responsible for planning, coordinating, and delivering bereavement services in accordance with hospice philosophy, Medicare Conditions of Participation, and organizational policies. The Bereavement Coordinator plays a vital role in ensuring families feel supported, heard, and cared for before and after a patient’s death.

Requirements

  • Bachelor’s degree in Social Work, Counseling, Psychology, Human Services, or a related field required.
  • Experience working with grief, loss, hospice, or end-of-life care strongly preferred.
  • Knowledge of hospice philosophy and Medicare hospice requirements is a plus.
  • Strong interpersonal, communication, and organizational skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary team.
  • Valid driver’s license and reliable transportation.
  • Compassionate, empathetic, and emotionally resilient.
  • Strong listening and counseling skills.
  • Excellent time management and documentation abilities.
  • Comfortable working with diverse populations and belief systems.
  • Ability to manage sensitive situations with professionalism and care.

Nice To Haves

  • Master’s degree in Social Work or Counseling preferred.
  • Experience working with grief, loss, hospice, or end-of-life care strongly preferred.
  • Knowledge of hospice philosophy and Medicare hospice requirements is a plus.

Responsibilities

  • Coordinate and oversee the hospice bereavement program, including assessment, planning, implementation, and documentation of bereavement services.
  • Provide grief education, emotional support, and counseling to families and caregivers before and after a patient’s death.
  • Complete bereavement risk assessments and develop individualized bereavement plans of care.
  • Maintain regular follow-up with bereaved families through phone calls, visits, mailings, and support groups for up to 13 months following death.
  • Facilitate or coordinate grief support groups, memorial services, and remembrance events.
  • Collaborate with the interdisciplinary hospice team to ensure continuity of emotional and psychosocial support.
  • Maintain accurate, timely documentation in the electronic medical record in compliance with regulatory standards.
  • Provide education and resources to staff and volunteers related to grief, loss, and bereavement support.
  • Coordinate and support bereavement volunteers, as applicable.
  • Ensure compliance with state, federal, and Medicare hospice regulations related to bereavement services.
