About The Position

The position involves providing age-specific care to patients and families serviced by Bridge Hospice. The role requires maintaining an assigned patient caseload within scheduled hours while minimizing overtime. The individual will identify and manage caregiver and family needs related to bereavement program services, ensuring exceptional bereavement care. Participation in the Interdisciplinary Team process regarding patient and family mental or psychosocial conditions and bereavement plans is essential. The role also includes communicating hospice services and benefits, discussing payment options, and utilizing educational materials or outside resources as needed. Documentation must be consistent with or exceed Bridge Hospice guidelines. The individual will plan, direct, monitor, and evaluate the Bereavement Program and its activities, facilitating caregiver support groups as appropriate. Assisting patients, families, colleagues, and oneself to cope with grief and loss while maintaining professionalism is crucial. The position emphasizes service excellence, compliance with organizational policies, and ensuring quality job performance and Medicare/Medicaid compliance.

Requirements

  • Good interpersonal skills, knowledge of family dynamics and counseling skills are essential.
  • Commitment to and knowledge of the philosophy, mission and goals of the agency, and the standards/regulations governing it.
  • Positive service-oriented interpersonal and communication skills required.
  • Ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient served.
  • Knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient status.
  • Ability to interpret the appropriate information needed to identify each patient’s requirements relative to their age-specific needs.
  • This position is classified 'at risk' for possible occupational exposure to blood borne pathogens (HBV, HIV, etc.)

Nice To Haves

  • Bachelor’s Degree in social work from an approved school of social work and state licensure are preferred.
  • Bachelor’s degree in related field with documented evidence of experience in grief or loss counseling.
  • Previous experience with terminally ill persons and/or hospice experience preferred.
