About The Position

BAYADA Home Health Care is currently seeking a compassionate Bereavement Coordinator to reliably serve patients in NJ and PA. This is a full-time position. At BAYADA, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Requirements

• Previous home healthcare experience preferred.
• Ability to travel to cases as assigned.
• A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.
• The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability.

Responsibilities

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Occasional on call after hours and/or weekends.
