About The Position

The Bereavement Coordinator coordinates the development and implementation of a plan of care as desired by the client and family. The Bereavement Coordinator completes bi-psycho-social assessments and provides appropriate social work and care coordination interventions. Serves as a resource to the community for bereavement issues. Performs other related duties as assigned.

Requirements

• Bachelor's Degree in Social Work or Associate's Degree with 4 years of relevant experience.
• 1 - 2 years of experience working with individuals who have experienced loss (preferred).
• 1 - 2 years of experience leading support groups (preferred).

Responsibilities
