We are hiring for a Bereavement Coordinator- Hospice at Assured Hospice of Port Angeles, part of LHC Group. This role is essential in providing bereavement services that are reflective of the needs of patients and their families. The Bereavement Coordinator is a qualified professional who supervises the provision of these services, ensuring they are delivered with compassion and respect for the diverse backgrounds of those we serve. The position requires proficiency in offering bereavement support to individuals of all races, ages, genders, sexual orientations, social classes, spiritual beliefs, and mental/emotional statuses. This is evidenced through observation and EGSS scores, which measure the effectiveness of the services provided. The Bereavement Coordinator will establish a comprehensive Plan of Care that addresses the bereavement needs of families, detailing the services to be provided and the frequency of service delivery for up to thirteen months following the death of a patient. Active listening and impactful suggestions are crucial during phone and home visits, adhering to LHC-required time guidelines. The role also involves leading and coordinating at least twelve effective sessions of Grief Support Group each calendar year, maintaining a Bereavement Binder that includes information and attendance sheets from various memorial services, grief support groups, and community presentations. Individualized and compassionate care is a hallmark of this position, with documentation in Visit Notes, Bereavement Risk Assessments, and Care Plans being essential to demonstrate the quality of care provided.
