This role is responsible for the development, coordination and implementation of specialized program services, providing program oversight and consultation on specific grant programs while providing training, support, and enrichment to assigned clients/families.

  • Develop, coordinate, deliver/implement programs specific to grant population and disability groups and family members.
  • Coordinate services for grant recipients according to SOW, disability group and treatment plans.
  • Coordinate programs with necessary staff supports to complete SOW.
  • Develop, coordinate, deliver/implement specialty programs including all activities specific to SOW implementation.
  • Participate in and complete trainings necessary to deliver service programs in support of the grant SOW.
  • Train other program staff for cross training and to implement/assist in delivery of program SOW.
  • Implement appropriate interventions and document patient’s responses in the medical record (as appropriate) using identified formats.
  • Identify thought patterns and behaviors associated with different types of psychiatric diagnoses and developmental ages and intervene appropriately utilizing the master treatment plan.
  • Transport and supervise patients to appointments and activities to implement grant SOW.
  • Answer the telephone and respond to all calls, including Helpline calls, in an appropriate and professional manner.
  • Conduct self in a professional manner (communication and appearance) with recipients, families, and other associates to include maintaining all the confidential aspects of the program.
  • Complete and maintain the quantity and quality of documentation to meet data requirements, regulatory and organizations requirements.
  • Create and grow collaborative relationships with other service providers in the community.
  • Bachelor's degree in a human service-related field such as psychology, sociology, social work, counseling, special education.
  • Valid WV driver’s license, valid vehicle insurance and a reliable vehicle.
  • State criminal background check and Federal (if applicable), as required for regulated areas.
  • Completion of a practicum/internship during the 4 year degree training in the human services field OR one year of on the job supervision at Healthy Minds.
  • Master's degree (BA/BS/MA/MS) degree in a human service related field such as psychology, sociology, social work, counseling, special education, etc.
  • Two years of post degree experience.
