About The Position

This position will be a member of a team of behavioral health providers in the Center of LGBTQ Health Equity at Chase Brexton Health Care and will provide direct behavioral health services in support of integrated primary care. Behavioral health will encompass prevention, intervention, and recovery from behavioral health issues and concerns, in addition to providing assessments and letters for gender affirmation surgeries (GAS) within the primary care setting across Chase Brexton Health Care.

Requirements

  • Minimum Master's Degree in Social Work
  • Provisionally licensed LMSW, LGPC; LCPC, LCSW-C, or Licensed Psychologist in the state of Maryland preferred
  • Experience in the provision of outpatient mental health services
  • Clinical experience with LGBTQ patients and community
  • Familiarity with open access model to care in community mental health and healthcare settings

Nice To Haves

  • Knowledge of LGBTQ 50 plus community and/or children and adolescents
  • Experience in LGBTQ behavioral care, ideally in a primary care environment

Responsibilities

  • Conducts brief, evidence-based and developmentally appropriate screenings
  • Conducts time limited assessments
  • GAS Assessments and surgical letters as needed (training provided)
  • Applies knowledge of evidence-based practice to individuals, couples, groups, and families
  • Provides emergency assessment of patients in crisis and facilitates psychiatric hospitalization if needed
  • Assesses discharge readiness throughout the course of treatment
  • Creates and implements integrated treatment plans
  • Independently evaluates treatment progress and modifies planning as indicated
  • Provides consultation to medical providers and other service providers
  • Participates in staff meetings and collaborative teamwork
  • Ensures progress notes are accessible to the patient's care team
  • Provides referrals for internal and external services
  • Organizes and maintains educational and referral materials
  • Engages patients and/or families who are not engaged in services through outreach and follow up
  • Documents all behavioral health services as directed by policy and standards
  • Adheres to departmental productivity standards and policy and procedures
  • Assesses for risk using a standardized measure
  • Completes all compliance assignments
  • Utilizes Electronic Health Record to support and improve integrated healthcare
  • Meets mandatory continuing education requirements to maintain licensure/certification
  • Performs other duties as assigned by supervisor
