Position Purpose: Provide continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participate as a team member in the delivery of addictions and mental health treatment services to patients and their families and facilitate the issues of recovery into their daily living situations.

Requirements

  • Master’s degree in counseling, psychology, social work, or related field
  • LCPC, LCSW, LMFT required
  • Experience in behavioral health treatment
  • Able to conduct individual, group and family counseling sessions
  • Provide case management, treatment planning, and discharge planning for clients

Responsibilities

  • Work hours prescribed by leadership and any additional hours deemed necessary for the responsibilities assigned.
  • Conduct individual, group, and family counseling and educational sessions for clients and/or family members.
  • Complete client psychosocial and individualized treatment plan within required time frame.
  • Analyze client clinical and case management requirements and develop actionable plans to meet them.
  • Provide comprehensive education to clients and their support network on treatment protocols, related health concerns, and steps to recovery.
  • Undertake primary case management responsibilities for designated clients.
  • Maintain accurate documentation in client service records, adhering to standards set by licensing, accreditation, and funder requirements.
  • Demonstrate leadership by serving as the central point of contact and representative of the site, and maintain open channels of communication with all staff and external stakeholders.
  • Establish and maintain a visible and supportive presence within the community to promote mental health awareness, provide resources, and foster connections with individuals seeking support.
  • Present workshops, lectures and training to clients, families, and staff and community groups.
  • Ensure accurate and timely completion of all required documentation in electronic health record.
  • Contribute actively as an integral member of the clinical team, fostering collaboration and inclusivity by participating in all team meetings, discussions, and activities aimed at enhancing client care.
  • Exercise confidentiality in keeping with the professional Code of Conduct and within the framework of the law.
  • Deliver exceptional customer service consistently to every customer.
  • Serve as a role model and demonstrate positive guest relations in representing the agency.
  • Assume other additional responsibilities as requested by leadership.
