Requirements

  • Master’s Degree in Social Work, Counseling, or a related field
  • Active LPC or LMSW license
  • At least 3 years of outpatient behavioral health experience
  • Strong skills in assessment, therapy, treatment planning, and crisis intervention
  • Comfortable working in a structured outpatient setting with medication -management collaboration
  • Passion for integrated, team -based care
  • Commitment to serving frail, underserved, and vulnerable populations

Responsibilities

  • Conduct comprehensive behavioral health assessments
  • Develop individualized treatment plans based on patient needs and goals
  • Provide therapy services for adults and/or children in an outpatient setting
  • Deliver crisis intervention and stabilization support when appropriate
  • Coordinate care with primary care providers and interdisciplinary teams
  • Help connect patients with community resources and additional behavioral health services
  • Monitor patient progress and modify treatment plans to support long -term success
  • Maintain accurate, timely documentation including progress notes and care plans
