Behavioral Health Specialist (LPC/LMSW)

Requirements
- Master’s Degree in Social Work, Counseling, or a related field
- Active LPC or LMSW license
- At least 3 years of outpatient behavioral health experience
- Strong skills in assessment, therapy, treatment planning, and crisis intervention
- Comfortable working in a structured outpatient setting with medication -management collaboration
- Passion for integrated, team -based care
- Commitment to serving frail, underserved, and vulnerable populations
Responsibilities
- Conduct comprehensive behavioral health assessments
- Develop individualized treatment plans based on patient needs and goals
- Provide therapy services for adults and/or children in an outpatient setting
- Deliver crisis intervention and stabilization support when appropriate
- Coordinate care with primary care providers and interdisciplinary teams
- Help connect patients with community resources and additional behavioral health services
- Monitor patient progress and modify treatment plans to support long -term success
- Maintain accurate, timely documentation including progress notes and care plans
