About The Position

Comprehensive Healthcare provides innovative behavioral health and integrated healthcare services to clients of all ages. The Behavioral Health Specialist II provides clinical services in a residential setting that help build independent living skills, improve recognition and management of symptoms and assist in gaining natural supports through development of social skills. Provides assessments, develops treatment plans, conducts groups and facilitates discharge planning. Works with others to assist the resident in achieving treatment plan goals and collaborates to ensure cross-system care. Individual therapy may be provided using evidence-based interventions.

Requirements

  • Master’s degree in social or behavioral science or related field.
  • NPI Registration
  • Must be eligible for registration as Agency Affiliated Counselor
  • Valid driver's license and vehicle liability insurance

Responsibilities

  • Conducts behavioral health groups following Comprehensive Healthcare’s guidelines and policies.
  • Leads groups using effective group facilitation skills that promote participation and interaction among group members.
  • Utilizes group process to develop insight, practice skills, and motivate client for change.
  • Follows lesson plan or curriculum and connects presented material to individual treatment plan goals.
  • Provides a combination of direct service and service coordination, with internal and external resources, to assist the client with achieving identified treatment plan goals.
  • Demonstrates a comprehensive knowledge of community services and resources (including peer, natural, and community supports) and actively encourages individuals to access them.
  • Advocates for the client when appropriate to attain services and benefits to further recovery goals.
  • Utilizes appropriate crisis intervention techniques for client stabilization.
  • Accurately screens for risk to self, others, property and the presence of grave disability and makes appropriate referrals.
  • Documents and reports and/or notifies as required by law.
  • Applies QPRT (Question, Persuade, Refer or Treat) principles in assessing risk and in safety and treatment planning.
  • Develops and documents appropriate safety plans.
  • Conducts family therapy following Comprehensive Healthcare’s guidelines and polices.
  • Demonstrates appropriate clinical skills and techniques for client population served to engage clients in meeting treatment and recovery goals as outlined in the Treatment Plan.
  • Provides specific evidence-based practice treatment consistent with training received.
  • Demonstrates and utilizes knowledge of normal aging process and age related changes.
  • Demonstrates an understanding of neurocognitive disorders and their impacts on functioning.
  • Demonstrates awareness of how medical issues can affect mental status.
  • Conducts individual therapy following Comprehensive Healthcare’s guidelines and polices.
  • Demonstrates appropriate clinical skills and techniques for client population served to engage clients in meeting treatment and recovery goals as outlined in the Treatment Plan.
  • Provides specific evidence-based practice treatment consistent with training received.
  • Completes all required intakes/assessments following Comprehensive Healthcare’s guidelines and polices for content, format and timeliness.
  • Obtains appropriate and essential information during assessment.
  • Establishes rapport with clients and obtains and documents the essential clinical data which is age, culturally and disability sensitive and specific.
  • Interprets clinical data to establish and substantiate medical necessity and diagnosis.
  • Develops treatment recommendations, case conceptualization and goals.
  • Demonstrates ability to minimize distractions while observing medication self-administration.
  • Demonstrates ability to locate and adhere to medication management policies and procedures.
  • Demonstrates ability to maintain cleanliness and hygiene.
  • Demonstrates ability to provide education, support and instructions during medication self-administration.
  • Practices the Five Rights of Medication Administration; Right Patient, Right Medication, Right Dose, Right Route and Right Time.
  • Demonstrates ability to order/re-order medications.
  • Demonstrates ability to document medication self-administration in Medication Administration Record (MAR).
  • Conducts and documents ongoing assessment of medication adherence, medication effectiveness, and side effects of medications reporting information to appropriate supervisor(s) and/or physicians as applicable.
  • Demonstrates ability to locate and complete Medication Error Reports according to policy.
  • Demonstrates ability to store and/or dispose of medications according to policy.
  • Conducts and documents ongoing assessment of clients understanding of need for medication and provides education as applicable.
  • Completes all required service notes according to Comprehensive Healthcare’s guidelines, policies and procedures regarding format, content and timeliness.
  • Language used in service notes is respectful, clear, descriptive, concise and client-centered.
  • Service notes accurately depict treatment modalities and interventions provided within the clinician’s scope of practice and according to treatment plan.
  • Service notes accurately reflect and depict client status and progress consistent with assessment and reassessment data, problem statements and treatment plan goals and objectives.
  • Conducts and documents ongoing assessment of client symptoms and their response to prescribed medications.
  • Provides support and education to encourage clients to take medications as prescribed.
  • Promotes illness self-management skills to assist clients with medication adherence, identifying warning signs and symptoms, and symptom management skills.
  • Monitors medication side effects and facilitates access to, and communication with, medical staff as needed.
  • Completes all required treatment plans following Comprehensive Healthcare’s guidelines and polices for content, format and timeliness.
  • Applies assessment, evaluation and reassessment data to the formulation of treatment plans.
  • Supports recovery principles by incorporating the individual’s and their family’s (when applicable) strengths, barriers and needs into the plan components.
  • Documents problem statements and treatment goals/objectives/ discharge plan that are written in clear and understandable language and includes client’s own voice.
  • Formulates measurable, achievable goals and objectives within the scope of treatment provided in collaboration with the client.
  • Describes clinical modalities/interventions that are reasonable and provided according to applicable Comprehensive Healthcare’s protocols.
  • Reassess treatment effectiveness and adapts treatment plan as needed in collaboration with the individual client.
  • Serves as a Protective Payee for assigned clients, tracks expenditures, and accurately maintains client budget sheets.
  • Promotes independent management of personal finances.
  • Monitors client LRA requirements and completes monthly LRA reviews.
  • Participates in the renewal of a LRA when a renewal is necessary.
  • Encourages clients to voluntarily participate in treatment.
  • Participates in the after-hours call coverage schedule rotation.
  • Responds to calls in a professional and timely manner.
