About The Position

Performs intake assessments on patients, coordinates care plans, and facilitates discharge planning within the Residential Treatment Center (RTC). Monday through Friday; 8 AM to 5 PM MAJOR RESPONSIBILITIES & DUTIES: Partners with intake team members to perform intake assessments, assist with collecting consent for treatment, release of information, and provide program orientation. Facilitates care for the patient through referral to appropriate resources in the community or within the Boys Town system. Provides leadership in the identification and acquisition of resources (local, state, and federal) for patients and their families. Participates in the multi-disciplinary RTC team by assisting with a variety of tasks to include, but not limited to, data collection/analysis/reporting (trends, youth served, funding, follow-up, etc.) answering referral calls/questions, coordinating activities/follow-up steps/procedures, and acting as a resource for physicians and therapists. Assumes the duties of the discharge planning coordinator. Will provide oversight of the discharge planning process. A daily review of the active patient discharge plans will be completed as needed. Reassess the discharge planning process on an on-going basis (i.e., at least quarterly), review discharge plans in closed medical record review to determine staff responsiveness to the patient’s post-discharge needs and review re-admissions for trends. Coordinates activities/follow-up to meet identified needs of mental health patients and their families. Maintains patient records, prepares complete, accurate, and timely reports, and monitors progress toward treatment goals. Complies with all applicable standards, statutes, rules, regulations, policies and procedures. Maintains working knowledge of relevant medical/legal issues that impact patient care. Participates in quality improvement activities through staff meetings, committees, continuing educational and professional activities.

Requirements

  • Ability to utilize a variety of computer programs and applications to manage work, familiarity with Windows and Office programs.
  • Ability to communicate concisely, orally and in writing, with patients, families, and staff and to collaborate with members of the health care, leadership team and community agencies to enhance care of the patient.
  • Knowledge of community resources.
  • Ability to work with diverse patient groups and their families.
  • Bachelor’s degree from an accredited school of social work required.
  • Certification, Licensure, or Provisional Licensure as a Social Worker in the State of NE required.
  • Minimum 21 years of age required.

Nice To Haves

  • Minimum of two years of experience working with the social services needs of the child and adolescent mentally ill population preferred.
  • Minimum of two years’ experience in utilization review and/or other healthcare related practices preferred.
