About The Position

Requirements

• Master's degree in social work required.
• At least one year of actual work experience in a hospital setting or an ongoing plan for consultation between the social worker and a qualified social worker must be developed.
• Must have knowledge of social and physical factors that affect functional status at discharge and knowledge of community resources to meet post discharge clinical and social needs.
• Currently licensed by the Nevada State Board of Examiners for Social Workers (LMSW).
• Must be able to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patients served on the assigned unit/department.

Responsibilities

• Responsible for the appropriate and timely discharge and transfer arrangements for patients referred to the Care Management Dept.
