About The Position
Behavioral Health Social Worker (LCSW) – Full Time Rotterdam & Latham Centers Monday–Friday, No Nights or Weekends Eddy SeniorCare is a program of all-inclusive care for the elderly, with two centers: one in Rotterdam and one in Latham. This is a unique model of care designed to help seniors avoid/delay nursing home placement and remain in the community for as long as possible. The program serves individuals 55 years of age and older in zip codes nearby both our centers. Eddy SeniorCare serves as both payer (we provide their Medicare and Medicaid insurance) and provider (offering a full range of services--from primary care providers, to home health care, transportation, adult day care, activities, social work, RN case management, nutrition, home delivered meals and medications and medical supplies, and much more!). Job Description: Works as an integral part of an interdisciplinary team to provide mental health services (counseling, resources, and education). Provides person centered, trauma informed therapy, brief counseling, and care coordination to participants and their families. Opportunity for group work in a Day Center setting.
Requirements
- Master's degree in Social Worker.
- LCSW License Required.
- Experience in behavioral health settings
- Strong assessment skills, including safety planning.

Responsibilities
- Provide person-centered, trauma-informed therapy, brief counseling, and care coordination to participants and families.
- Offer mental health services including counseling, resources, and education.
- Facilitate group sessions in our Day Centers.
- Collaborate with medical and social service professionals to ensure comprehensive care.
