About The Position

In this role, you will assume responsibility to lead our team in providing quality patient care. You will assess, plan, implement and evaluate patient care from admission to discharge and you will be responsible to direct, assign and supervise ancillary staff team members in providing the appropriate level of care in the most efficient manner. You will also act as a role model to staff members. As a Registered Nurse (RN) for our team, you will be responsible for completing assessments, administering medications/treatments, observing patient’s progress, and evaluating therapies for clinical outcomes and effectiveness. You will collaborate with physicians and other medical professionals to develop an optimal health care strategy plan for each patient, and support and educate patients and their loved ones to achieve maximum outcomes. The role emphasizes a positive work environment, evidence-based practice, and a comprehensive team approach.

Requirements

  • Graduate of an accredited school of nursing
  • Current Massachusetts RN licensure
  • BCLS required
  • Vaccinated against influenza (flu) as a condition of employment

Nice To Haves

  • Previous Diabetes Care experience preferred
  • Bachelor of Science Degree Preferred
  • Certified Diabetes Educator

Responsibilities

  • Collect health data and determine nursing diagnoses
  • Perform complete and thorough assessments of the patient's physical, functional, spiritual, cultural, psychosocial, nutritional and pain status
  • Identify needs of the patient/family
  • Conduct reassessments according to current nursing standards
  • Consistently assess patient/family discharge planning and educational needs
  • Develop a plan of care that identifies expected outcomes individualized to the patient and prescribes interventions to attain outcomes
  • Develop an individualized and flexible plan of care for each patient utilizing appropriate nursing standards, incorporating the patient and family in the development of the plan
  • Consult and collaborate with other disciplines to meet identified patient/family needs
  • Develop and implement patient/family teaching plan
  • Identify individualized goals for each patient based on current knowledge and skills
  • Implement the interventions to attain expected outcomes
  • Implement the plan of care following the actions specified
  • Document progress towards goals established in the plan of care utilizing the appropriate forms and or tools
  • Administer medications safely and in accordance with hospital policies and procedures
  • Demonstrate knowledge of dosage ranges, therapeutic indications and side effects for medications administered to the patient
  • Apply clinical judgment in the appropriate administration of "PRN" medications
  • Utilize the pain scale as part of the pre and post pain assessment with each dosage administered
  • Adhere to Patient Care Services and Areas of Practice policies, procedures, standards of care and code of ethics
  • Evaluate the patient's progress toward attainment of outcomes
  • Evaluate the plan of care, reviewing and revising as needed, anticipating and intervening in potential problems/complications before they occur
  • Evaluate the patient/family teaching plan, reviewing and revising as needed
  • Communicate with other disciplines, as appropriate in regards to evaluating the effectiveness of the plan of care
  • Revise plan of care as appropriate
