Liberty Resources is seeking applicants for the position of Behavioral Health Registered Nurse (RN) for Liberty Resources Integrated Health Care in our Syracuse, NY location. About Us: Liberty Resources Integrated Health Care is a Certified Community Behavioral Health Center in Syracuse, serving children, families and adults with behavioral health challenges including co-occurring substance abuse. Behavioral Health Registered Nurse (RN) Position Summary: The Behavioral Health Registered Nurse will function as a member of the Integrated Health Care Clinical Team in assisting with a full range of nursing/medical services for clients. The Behavioral Health Registered Nurse will perform tasks and responsibilities within the framework of health education, client counseling, health monitoring, assessment, medication administration, and the provision of supportive mental health care including crisis intervention. The RN will function within the scope of practice of their license and community standards of care.

Requirements

  • Must be licensed as a Registered Nurse and currently registered to practice nursing in New York State.
  • Must have graduated from an accredited college or school of nursing.
  • Must possess a Bachelor’s Degree or higher and have two years of professional clinical nursing experience, with at least one year of experience providing care to individuals diagnosed with mental illness, mental retardation, developmental disabilities, and/or alcohol or substance abuse.
  • Must possess a valid driver’s license and access to reliable transportation.
  • Must have eligibility for full and unconditional participation in the Medicare and Medicaid programs.

Responsibilities

  • Provides assessments, consultations, medical intervention, and clinic based services for clients to determine current health needs and to initiate and/or contribute to overall plan of care.
  • Provides medical crisis intervention/counseling as needed. Completes medication reviews, progress reports, and comprehensive nursing documentation in the electronic medical record in consultation with prescribers as needed.
  • Develops a comprehensive care plan to assist clients in meeting identified health goals, provides direction to staff in support of achieving the care plan, and enhances patient safety per program guidelines.
  • Provides education and training to clients and staff on health-related topics like preventative care, infection control, crisis management, medication management and other health topics as needed. Oversees LPN in ongoing educational efforts.
  • Communicates and consults with medical practitioners, Agency supervisors and other professionals in regards to client’s conditions, communicable diseases, responses and needs to promote ongoing management of the client’s health needs and within specified policy timeframes.
  • Coordinates services with other disciplines, agencies, and providers to meet client's needs in an efficient and cost effective manner. This includes managing prior authorizations, scripts and medication refills, and managing incoming and outgoing communications with outside agencies.
  • Formulates and revises care plans to consistently reflect the needs of the client in managing health problems per program guidelines or when patient condition warrants.
  • Provides nursing care, prescribed treatments, administers medications, and obtains specimens for lab work and other services in accordance with plan of care that is consistent with the medical regime and performed in an efficient and cost effective manner.
  • Responds to staff and client mental and physical health emergencies/crises at the clinic.
  • Documents assessments, discussions regarding advanced directives, conferences, assessments, care plans, summaries per agency standards.
  • Ensures that all training requirements are continually met per Agency standard and program policy. Maintains required professional license and program certifications as required for ongoing practice.

  • Carries out Agency policies and procedures. Periodically reviews program manuals to ensure current medical and health policy is maintained. Assists with developing and amending agency policies and procedures.
  • Completes all necessary written reports, statistics, case files and summaries in a timely manner and in accordance with program and agency standards. Assists in maintaining accurate communication and information transfer with health care providers in the community.
  • Work at least one day per week until 7:00pm or clinic closing as determined by supervisor.
  • Carries out agency policies and procedures.
  • Perform other related duties as assigned.
