Position Summary: The primary responsibility of a Behavioral Health Professional (BHP) is to provide rehabilitative and community support services for persons with cognitive impairments and functional limitations focusing primarily upon social, behavioral and functional skills as well as independent living skills. Essential Functions: Provide rehabilitative and community support services as outlined in the Individualized Treatment Plan (ITP) to increase skill development and adaptive behaviors including but not limited to the following: Activities of Daily Living Social Skills Communication Skills Safety Awareness Behavior Modification Coping Skills Community Integration Provide documentation of services Serve as a valuable member of the Interdisciplinary Team to: Assist in the development/ revision of the ITP Provide feedback related to goal progression Assist in identifying barriers to care Assist parents/ families in understanding child behavior Work collaboratively with other members of the team Comply with mandatory reporting practices Participate in monthly supervision with the Child Services Coordinator Schedule: Flexible Scheduling Weekdays & Weekends available
