About The Position

As a Partner in Wellbeing, the Crisis Behavioral Health Practitioner provides mobile mental health crisis services in the form of screening, crisis assessments, interventions, and stabilizations to clients with mental illness or children and families experiencing emotional disturbance. Services are provided under the oversight and direction of a mental health professional in the form of instruction, support, and assistance for the purpose of promoting psychiatric stability, social competencies, personal and emotional adjustments, and independent living skills.

Requirements

  • Relevant coursework by completing at least 30 semester hours or 45 quarter hours in Behavioral Sciences or related fields and having at least 2,000 hours of supervised experience in the delivery of services to adults or children with mental illness, substance use disorder, etc.
  • Qualified through work experience 4,000 hours of experience in the delivery of services to adults or children with mental illness, substance use disorder, etc.
  • Must pass a criminal background check through MN DHS NetStudy.
  • Access to a personal vehicle and the ability and willingness to use the vehicle in carrying out the position’s responsibilities.
  • Valid driver’s license and insurable under the agency’s automobile insurance policy.
  • Free from substance use problems for no less than 2 years.

Responsibilities

  • Complete crisis assessments at an individual’s home or in the community when activated. On some occasions the practitioner may be able to provide telehealth when in-person options are not possible.
  • Initiates or completes co-response to crisis activations when appropriate. (For example, with law enforcement or social services representative)
  • Develop intervention plans to support individuals in their ability to manage/cope with their immediate stressors including safety planning.
  • Create stabilization plans to support individuals in successfully competing activities that support their stabilization goals.
  • Provides services as outlined in the client’s crisis intervention / stabilization plan in the home, community, school, or office to help clients cope and stabilize symptoms of their mental health condition.
  • Provide adequate documentation of services.
  • Complete initial and ongoing mental health outcomes measurements as well as develop and implement client cantered treatment plans.
  • Participates and works collaboratively in all care coordination activities concerning clients including Interdisciplinary Care Coordination meetings.
  • Participate in clinical & administrative supervision as well as all team meetings.
  • Focuses on integrated health (physical, mental, substance use, and oral) care, ensuring clients are referred to any necessary internal and/or external services to support their overall well-being.
  • Complete client and community outreach as directed, including post-vention support.
  • Participate and represent Alluma at community events.
  • Performs additional duties as assigned or requested.
