About The Position

What You’ll Do Provide direct patient care, including assessments and care plan development Manage a caseload and coordinate care across the clinical team Administer medications and treatments per physician orders Monitor patient progress and adjust care plans as needed Educate patients and caregivers on care and self-management. Communicate with physicians and care teams to ensure continuity of care Supervise and support HHAs and LPNs Complete accurate and timely documentation (OASIS, visit notes, orders) Ensure compliance with policies, regulations, and HIPAA standards What You Bring Active RN license Graduate of an accredited nursing program (BSN preferred) Home health or related behavioral health clinical experience Strong communication, organization, and critical thinking skills Current CPR certification Valid driver’s license and reliable transportation Physical Demands & Work Environment Frequent standing, walking, sitting, and use of hands Regular bending, kneeling, and reaching Occasional lifting and repositioning of patients Ability to lift/push up to and over 25 lbs as needed Exposure to clinical environments and infectious materials

Requirements

• Active RN license
• Graduate of an accredited nursing program (BSN preferred)
• Home health or related behavioral health clinical experience
• Strong communication, organization, and critical thinking skills
• Current CPR certification
• Valid driver’s license and reliable transportation
