About The Position

Primary Duties and Responsibilities The Home Health/Hospice Nurse I (RN) role provides direct patient care in accordance with the Nurse Practice Act to assigned patients. The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care. This includes providing physical assessments, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned. Utilizes the nursing process based on evidence-based best practices to meet patient care needs. Evaluates and adjusts the nursing care process as necessary with consultation as needed, and confers with the appropriate resource personnel when making ethical/legal/medical dilemma decisions. Performs other duties as assigned. Behavioral Health Nurse (RN): Utilizes medical/ psychosocial model to individualize care plans based on Physician orders for patients with depression, anxiety, or dementia, with the goal of keeping patients in their homes and out of the hospital. Performs comprehensive behavioral health evaluations of mental status, mood, and behavior.

Requirements

  • One (1) year of home health care experience preferred.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Current licensure as a Registered Nurse in the State of California.
  • Current Driver's License
  • Current BLS certification prior to clinical rotation.
