About The Position

The Behavioral Health Navigator is a Licensed Clinical Social Worker (LCSW) who plays a crucial role in supporting patients and their families during hospital stays, particularly those dealing with substance use disorders. This position involves assessing psycho-social and economic barriers to healthcare goals, providing psychotherapy, and collaborating with a multi-disciplinary healthcare team to ensure comprehensive support and continuity of care for patients.

Requirements

• Master's Degree in Social Work or Counseling
• Two years' experience in clinical social work
• State of Florida Licensed Clinical Social Worker (LCSW)

• Experience in providing psychotherapy

Responsibilities

• Perform psycho-social assessments on patients and families and provide psychotherapy to those coping with opioid addiction issues.
• Participate in the development of programs to support patients and families dealing with opioid addiction and substance use disorders.
• Effectively communicate with patients, families, staff, physicians, and community leaders to enhance support for patients.
• Collaborate with the multi-disciplinary team to document psycho-social determinants for safe discharges.
• Maintain comprehensive community resources and networks to assist patients and families transitioning back to the community.
• Serve as a consultant and educator to the multi-disciplinary team regarding psycho-social components of illness and hospitalization.
