About The Position

Heritage Behavioral Health Center is seeking passionate, mission-driven professionals to join our team as a Behavioral Health Navigator! The Behavioral Health Service Navigator serves as the first point of contact for individuals seeking low‑intensity, office-based case management support. This position focuses on walk-in support, needs assessments, care coordination, service linkage, and ongoing follow-up to ensure clients receive timely access to behavioral health and community resources. You will maintain a caseload, coordinate with internal teams and external agencies, and provide client-centered support to help individuals achieve their goals.

Requirements

  • High School Diploma or GED + 5 years supervised mental health or social service experience required
  • Must be 21 years of age or older
  • Valid driver’s license, reliable transportation, and proof of auto insurance
  • Understanding of behavioral health systems, case management principles, and community resources
  • Strong communication and engagement skills
  • Ability to create individualized plans and track progress
  • Strong organization and time‑management skills
  • Ability to work independently and collaboratively
  • Crisis intervention knowledge and client-centered approach
  • Ability to lift 10 pounds

Nice To Haves

  • Bachelor’s degree in social work, psychology, education, or human services preferred

Responsibilities

  • Conduct intake needs assessments and ongoing evaluations of client strengths, needs, and goals
  • Develop and monitor individualized service plans
  • Maintain an active caseload and complete timely follow-up and documentation
  • Provide support, advocacy, and linkage to community resources, healthcare, and social services
  • Adjust service plans based on client progress
  • Facilitate referrals to internal programs and external agencies
  • Participate in team meetings, case reviews, and professional development
  • Offer short-term or ongoing office-based support based on client needs
  • Conduct proactive follow-ups and barrier identification
  • Complete all documentation and tracking accurately and promptly
  • Perform other duties as assigned or negotiated with supervisor
