The Behavioral Health Service Navigator serves as the first point of contact for individuals seeking low‑intensity, office-based case management support. This position focuses on walk-in support, needs assessments, care coordination, service linkage, and ongoing follow-up to ensure clients receive timely access to behavioral health and community resources. You will maintain a caseload, coordinate with internal teams and external agencies, and provide client-centered support to help individuals achieve their goals.
