About The Position

The Behavioral Health Navigator at Hawaii Medical Service Association (HMSA) is responsible for screening behavioral health members and coordinating their care. Requirements

  • Reliable transportation for in-person meetings.

Responsibilities

  • Conduct telephonic screenings of behavioral health members using diagnostic tools such as PHQ-9 and GAD.
  • Communicate effectively with HMSA members, physicians, case managers, and disease management team members.
  • Coordinate with colleagues to ensure continuity of care for members in the assigned work queue.
  • Triage members to the appropriate behavioral health program after assessment.
  • Complete and file forms and input data into HMSA systems.
  • Provide navigation, referrals to community resources, and educational materials.
