Requirements

  • Lead with professional credibility. Hold a Master’s degree in Social Work (MSW) from a CSWE-accredited program and an active California license as an LCSW or LMFT—demonstrating the highest professional standards in your field.
  • Bring depth and experience. Offer at least 5 years of progressive social work experience, including 3 or more years in a leadership or management capacity, ideally within healthcare, PACE, or senior-care environments.
  • Understand complex populations. Possess deep expertise in supporting older adults, high-risk seniors, and individuals with complex medical, behavioral, and psychosocial needs.
  • Think strategically and act decisively. Approach challenges with a solutions-oriented mindset—balancing long-term vision with the day-to-day operational realities of care delivery.
  • Communicate with clarity and care. Exhibit outstanding verbal and written communication skills, fostering understanding and alignment among participants, families, and team members.

  • Be fully present. This role is 100% onsite. You’ll lead your team and engage directly with participants and colleagues at our PACE center.
  • Enjoy a consistent rhythm. Standard schedule is Monday through Friday , allowing for dependable structure while making space for meaningful daily connections.
  • Stay adaptable. Flexibility is key. Occasional on-call availability may be required to support participant or operational needs.
  • Travel with purpose. You may travel between nearby sites as needed to provide coverage, mentorship, and operational support.
  • Work independently and confidently. Must be able to perform essential tasks independently, including using computers and software applications, answering phones, and managing documentation in the EMR.
  • Move with purpose. Frequent walking, standing, and movement throughout the center are part of daily operations.
  • Maintain safety and preparedness. Standard precautions apply, with appropriate PPE and infection control practices followed at all times.

Responsibilities

  • Lead with purpose and direction. Oversee the Social Work department by setting clear goals, priorities, and performance standards that align with myPlace Health’s mission and PACE program requirements.
  • Inspire and empower your team. Supervise and mentor a group of talented Social Workers and Mental Health Counselors, creating an environment that encourages accountability, teamwork, and continuous professional growth.
  • Build strong operational rhythms. Manage team schedules, coverage, and onboarding processes while driving performance management and skill development.
  • Use data to drive impact. Leverage analytics and key performance indicators to identify trends, inform decisions, and lead improvements in service delivery.
  • Champion clinical excellence. Ensure timely, high-quality completion of biopsychosocial assessments, individualized care plans, and social work interventions that reflect compassion and regulatory compliance.
  • Streamline processes for consistency. Establish and refine workflows, policies, and procedures that ensure consistent, participant-centered care across multiple sites.
  • Guide through complexity. Support high-acuity cases, crisis intervention, and transitions of care to safeguard participant well-being and stability.
  • Serve as a trusted expert. Provide guidance on trauma-informed care, advanced care planning, grief support, and addressing social determinants of health.
  • Lead by example in care delivery. Maintain a small participant caseload to stay closely connected to the day-to-day realities of social work practice—ensuring your leadership remains grounded, informed, and participant-centered.
  • Uphold integrity through compliance. Ensure all practices meet CMS, DHCS, and PACE standards, maintaining the highest levels of ethical and regulatory accountability.
  • Elevate quality and performance. Oversee EMR documentation for completeness and accuracy while leading audits, surveys, and corrective action planning to drive continuous quality improvement.
  • Collaborate beyond boundaries. Partner with Nursing, Therapy, Enrollment, and Center Operations to ensure seamless, interdisciplinary care for every participant.
  • Advocate for alignment and integration. Represent the Social Work function in organizational planning, operational readiness, and strategic growth initiatives.
  • Steward resources wisely. Manage resource allocation and vendor partnerships related to social work services to support efficient and effective care delivery.
  • Adapt and contribute. Perform other responsibilities as needed in support of organizational goals and participant-centered outcomes.
