Summary Develops and implements treatment plans, provides therapeutic interventions, and facilitates referrals both within the LVHN continuum and throughout the community in order to address patients' on-going needs with substance use disorders.

Requirements

  • Bachelor’s Degree Social Work, Psychology, Counseling, Nursing, or related health/mental health field.
  • 2 years Experience in a health/mental health-related field.
  • Ability to maintain current knowledge of community agencies and support services.
  • Demonstrates ability to develop a relationship with patient/family that supports their recovery.
  • Demonstrates an understanding of multi-cultural issues and incorporates these into counseling with the patient/family.

Nice To Haves

  • Master’s Degree Social Work, Psychology, Counseling, Nursing, or related health/mental health field.
  • Ability to implement trauma-informed care.
  • Experience with implementing motivational interviewing.

Responsibilities

  • Completes timely and comprehensive documentation of assessments, interventions, and interactions.
  • Maintains case records in accordance with regulatory standards and requirements.
  • Provides on-going support, facilitates adjustments to identified problem, and fosters independence for problem resolution.
  • Supports the care team and patient with implementing their recovery management plan.
  • Utilizes therapeutic techniques targeted at restoring patient to his/her optimal level of functioning.
  • Facilitates solution-focused discussions between the patient and care team that address financial barriers related to recovery.
  • Participates in regular interdisciplinary team meetings.
  • Cultivates and develops collaborative relationships with community organizations to create a seamless recovery management plan.
  • Maintains knowledge of current trends via literature and through continuing education.
