Title: Behavioral Health Inpatient Case Manager License Required: RN or Social Work License Salary: $30-40/hr Location: Pocatello, ID (onsite) Schedule: Day shift with every 3rd weekend required. As PMC’s Behavioral Health Inpatient Case Manager, your focus will be on the 3Cs; Care, Community and Compliance.

Requirements

  • License in social work or Registered Nurse license required.
  • Two or more years of previous experience in all aspects of social service with an emphasis in psychiatric work preferred.
  • Current certification/training in the management of disruptive and assaultive behavior.
  • BLS
  • Handle with Care will be provided
  • Basic computer knowledge
  • Strong interpersonal skills

Responsibilities

  • Provide the best possible patient care
  • Provides assessments, treatments, and case management to patients and families.
  • Completes psychosocial assessments on all new admissions.
  • Participates in discharge planning, arranges for follow-up care, and develops mechanisms for exchange of appropriate information with sources outside the hospital.
  • Plans and coordinates therapeutic program services on the behavioral health program unit.
  • Responsible for leading the treatment team.
  • Educates patients and families on signs and symptoms of illness to prevent re-hospitalization and improve disease management skills.
  • Participates in discharge planning, arranges for follow-up care, and develops mechanisms for exchange of appropriate information with sources outside the hospital.
  • Maintains performance improvement activities within the department and participates in quality improvement activities.
  • Serves as a resource person to healthcare professionals concerning psychosocial problems and community resources.
