About The Position
Woodfords' Behavioral Health Homes (BHH) team is searching for full-time (32-40 hours per week) BHH Coordinators to join our fun and inclusive team in Westbrook, ME! Position Summary The Behavioral Health Home (BHH) Coordinator plays a vital role in supporting children with disabilities and their families. This position focuses on providing advocacy, guidance, and connections to community-based services that promote family empowerment and long-term self-sufficiency. Support is delivered through comprehensive assessments, care plan development and monitoring, and active participation in family and team meetings.
Requirements
- Bachelors Degree in Social Work or related Human Service field (LSW Preferred)
- At least (3) three years experience in the field
- Knowledgeable about regulatory guidelines
- Valid driver's license and have use of a vehicle
Responsibilities
- Conduct regular visits with children and families in various settings to evaluate functioning, monitor progress, assess quality of life and safety, and address concerns.
- Maintain strong knowledge of local resources and assist families with appropriate referrals.
- Complete strengths and needs assessments and facilitate wraparound family meetings as needed.
- Act as a liaison between families and service providers (e.g., schools, behavioral health agencies, consultants) to ensure coordinated, adequate care and identify gaps in services.
