Work Shift: Conducts thorough assessments to evaluate clients' mental health status, behavioral issues, and psychosocial needs, utilizing standardized risk assessment tools and techniques to gather comprehensive information. Collaborates with patients, families, community support, and healthcare providers to develop individualized care plan based off assessment and tailored to both medical and psychosocial needs. Communicates and collaborates with those included in patient care team including but not limited to Physicians, unit nurses, social workers, security personal, and outside hospitals and agencies to coordinate and expedite the patient through the continuum of care management. Provides immediate crisis intervention, including de¬ escalation techniques, brief counseling, safety planning to help stabilize patients experiencing acute psychiatric crises, including but not limited to those with suicidal ideation, severe anxiety, psychosis, or substance induced issues. Educate and provide guidance to patients and their families about mental health conditions, assessment and treatment process, and available community resources. Facilitate referrals and/or resources to appropriate inpatient of outpatient mental health services ensuring smooth transition of continued care to address individualized needs. Maintain accurate and timely documentation of patient assessments, treatment plans, and outcomes in compliance with the hospital policies and regulatory standards in the electronic medical record (EMR) system. Advocate for patient's needs and rights, ensuring patients are given culturally competent and respectful care within the healthcare system.
