About The Position

Join our friendly, professional, and supportive organization where you can make a difference in someone's life every day! We are looking for part-time staff to provide direct adult rehabilitative support services to consumers in community-based transitional housing residences (Group Home). You will be responsible for on-site supervision, counseling, advocacy, and assisting consumers in achieving their goals as outlined in their individual rehabilitation plans. Your role will involve building consumer skills and assisting with activities of daily living, including personal hygiene, household chores, money management, community involvement, and medical case management. You will also provide crisis intervention, medication supervision, and maintain life safety documentation and standards.

Requirements

• Bachelor's degree in Mental Health or a related discipline; OR Licensed R.N.; OR two years of college plus two years of related work or life experience; OR licensed as a Practical Nurse (LPN) plus two years of related mental health experience; OR a High School diploma and four years of related mental health experience.
• Must possess a valid driver's license with a good driving record.

Nice To Haves

• Bilingual (English/Spanish) preferred, but not required.

Responsibilities

• Provide direct adult rehabilitative support services to consumers in community-based transitional housing residences.
• On-site supervision, counseling, and advocacy for consumers.
• Assist consumers in achieving their goals as outlined in their individual rehabilitation plans.
• Build consumer skills and assist with activities of daily living, including personal hygiene, household chores, and meal preparation.
• Support money management, community involvement, and recreational skills.
• Assist with medical case management and provide transportation in the agency vehicle.
• Provide crisis intervention and medication supervision.
• Maintain life safety documentation and standards, including fire drills and safety inspections.
