Conducts behavioral health evaluations to determine most appropriate treatment for adult patients starting at age 18 years, who have graduated high school and older. Hackettstown Medical Center has been providing care to the community since 1973, primarily serving Warren, Sussex and Morris Counties in New Jersey. From preventive services and outpatient testing, to care for minor injuries and life-threatening illness, we offer a wide range of services to keep our local population healthy. In addition, we provide education, screenings, support groups and wellness programs for people of all ages through our Center for Healthier Living. At Atlantic Behavioral Health, we know behavioral health plays a significant role in the healing process – not just for patients dealing with mental health disorders or substance abuse, but also for those recovering from injuries, surgeries, diseases, and other conditions. We help them cope with the stress and emotional challenges caused by such physical ailments. Our behavioral health team features highly trained psychiatrists, psychologists, social workers, and licensed clinicians. We provide a variety of mental health services on an inpatient and outpatient basis, including intensive outpatient programs.

Responsibilities

Conduct initial biopsychosocial assessments for patients seeking acute behavioral health services to determine need for treatment of behavioral issues.
Complete and analyze evidenced based screening tools and assessments.
Provides individual and group counseling for patients.
Along with patient, develop treatment plans, including clear problem statement, treatment goal(s),with measurable, relevant objectives for a range of behavioral health concerns.
Manage case load comprised of moderate to high acuity patients.
Able to conduct multiple psycho-educational, therapeutic and skills groups and individual therapy services.
Collaborate with other mental health and educational professionals to coordinate care.
Maintain accurate and up-to-date patient documentation records.
Work within a multi-disciplinary team to evaluate patient progress and adjust treatment plans as necessary.
Participate in ongoing professional development activities to stay current on best practices in the field of behavioral health.
Complete all mandatory AHS trainings and competencies as assigned.
