This role has no on-call duties and the ability to work a 4/10’s schedule. You will also receive a CME budget and reimbursement for license renewal. This position is eligible for our $1000 sign on bonus. The Behavioral Health Clinician is a member of the Metropolitan Pediatrics clinic staff, working in tandem with the clinic Medical Home care team to conduct assessments to evaluate the problem and utilize psycho/social health integration tools appropriate for the patient’s illness, injury, disability, or current health status. The Behavioral Health Clinician will help patients and families solve and cope with problems in their everyday lives to improve overall health outcomes in the integrated care setting. The diverse responsibilities of this role include timely provision of behavioral health therapy and care coordination through direct contact with patients and their families, assisting with behavioral health referrals and prior authorizations, providing patient and team education and community outreach.

Requirements

  • Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) or Licensed Professional Counselor (LPC) in the state of Oregon required.
  • 1-3 years of experience in a clinical setting required.
  • Transferrable experience considered.
  • Knowledge and understanding of community resources, and effectively accessing them.
  • Understanding of and experience working with PCPCH (Patient-Centered Primary Care Home) standards.
  • Proficiency in Microsoft Excel and Word required.
  • Experience working with an Electronic Medical Records system required with preference for Epic-specific experience.
  • Experience with interpretation of a variety of patient population reports.

Responsibilities

  • Act as a member of a robust multidisciplinary team to provide crisis intervention, brief therapy, case consultation, resource connection, care coordination, and program development.
  • Provide support for patient procedural distress.
  • Identify and assess behavioral health needs as part of a multidisciplinary primary care team, billing appropriately for services rendered.
  • Engage and activate patients and families in their care.
  • Work collaboratively as a primary care team member both preventatively and to meet acute patient needs, providing ongoing support as needed.
  • Ability to quickly build rapport, assess patient needs and provide brief clinical interventions.
  • Help observe and improve care team function and relationships via ongoing professional education, collaboration, and contribution.
  • Communicate effectively with other providers, staff, and patients.
  • Provide efficient and evidence-based care that meets the needs of our primary care population.
  • Provide culturally responsive, whole person, and family-oriented care.
  • Understand, value, and adapt to the diverse professional cultures of an integrated care team.
  • Provide timely behavioral health care coordination through direct contact with patients, their families, and community partners.
  • Provide patient, parent and team education and community outreach.
  • Contribute to ongoing program development and outcome research.
  • Assist in practice improvement efforts through participation in committees and provider meetings.
