About The Position

The Integrated Behavioral Health Program provides a collaborative framework for an integrated team-based process in the outpatient primary or specialty care clinics. The Behavioral Health Clinician (BHC) provides brief behavioral health interventions for patients with mental health needs and chronic illnesses who’s emotional and/or behavioral issues negatively impact their disease management. The BHC’s role is to support the ongoing behavioral health interventions of the primary care provider and the focus is to resolve problems within the primary care service context. The referring primary care provider, at all times, remains the overall care manager. This position will provide screening, triage, case management, and clinical intervention services to individuals who are receiving care in the primary or specialty care clinics. BHC will provide consultation and assist providers on the management of behavioral concerns in primary care and assist in the triaging and managing of emergent referrals.

Requirements

  • Master’s Degree in Social Work from a School of Social Work approved by the Council on Social Work Education.
  • Maintain current LMSW license in Idaho.
  • Licensed as an LCSW or working towards clinical certification
  • Masters degree in counseling from an accredited university program.
  • Maintain current LPC license.
  • Licensed as an LCPC or working towards a clinical certification.
  • Comfortable working with variety of individuals with co-morbid conditions.
  • Able to work in an interdisciplinary team.
  • Excellent verbal and written communication skills.
  • Able to read, write, and speak the English language.
  • Able to work with clients with limited English proficiency.
  • Able to make independent decisions when circumstances warrant such action.
  • Able to organize work.
  • Able to exchange information and communicate with the client, any available family members, other providers, and community agencies.
  • Able to work with ill, disabled, emotionally upset and sometimes hostile clients.
  • Able to take initiative and problem solve.
  • Able to use computers proficiently and electronic charting will be required.

Responsibilities

  • Service as a core member of the health care team within primary or specialty care clinics.
  • Primary job function is to provide brief clinical intervention to a caseload of primary or specialty care patients per established standards and guidelines.
  • Functions include engagement and relationship building, assessment, treatment planning, therapeutic interventions, interdisciplinary coordination, coordinating discharge plans, and monitoring referrals.
  • Secondary function is to assists with resource location and advocating for benefits.
  • Provide brief intervention therapy for clients who are not in need of longer-term mental health services or who present in the primary care environment who require more immediate attention.
  • Interventions include motivational enhancement counseling, adjustment to illness, treatment adherence issues, behavior modifications, stress management and psycho-education.
  • Provide mental health triage for clients referred by other providers in the primary care setting and provide crisis intervention for clients with more emergent behavioral health needs.
  • Effectively collaborate with health care providers within the Valor Health System and the larger community to meet the needs of mutual clients, including coordinating interdisciplinary case conferences.
  • Assist patients with transportation issues that impact treatment adherence.
  • Appropriately refer clients to financial advocate, community resources and consistently monitor the outcomes of said referrals.

  • Provide initial diagnostic evaluation and make determination for the need and level of care for treatment.
  • Responsible for completion and updates to clinical diagnostic assessment, treatment planning and readjustments to treatment plan and goals.
  • Coordination with primary care provider and psych medication management provider as needed.
  • Must be eligible to participate in Third Party billing.
  • Complete documentation of services in a legible manner in accordance with established current policy and payor guidelines.
  • Facilitate groups designed to engage clients in treatment, especially in the areas of disease management, coping skills and behavior change.
  • Regularly attend staff meetings, team meetings, in-services, etc.
  • Actively participate in performance improvement activities within the agency and team.
  • Appropriately perform other activities not described above that are deemed appropriate by the agency
