About The Position

Put your education and experience as a counselor to work at our family health center. This is a unique opportunity to provide care in an integrated behavioral and primary health care setting. Chestnut Health Systems™ is a leading provider of integrated health care services. We provide fully integrated care for all our patients by combining behavioral health care services with our community-based primary care health centers. Chestnut also conducts research and training throughout the U.S.A. This position may be eligible for a sign-on bonus! Schedule: Mon-Fri, 8am to 5 pm, No Weekends, or Holidays. The anticipated starting pay for new hires for this position is between $44,400 - $50,575 annual salary. There are several factors taken into consideration in determining base pay, including but not limited to: job-related qualifications, skills, education, experience, local market conditions, and internal equity

Requirements

  • Must have a Masters’ or doctoral degree in counseling and guidance, rehabilitation counseling, social work, psychology, pastoral counseling, marriage and family therapy or related field and operate under the direct supervision of an LPHA in the state of Illinois.
  • Valid driver's license, private automobile insurance and ability to be insurable, preferred.
  • Requires basic computer skills and experience to include MS Word & Excel, videoconferencing, email, and an electronic medical record (EMR).

Responsibilities

  • Provide behavioral health services to a diverse client population across the lifespan.
  • Deliver short-term, evidence-based interventions including solution-focused therapy, Motivational Interviewing, Cognitive Behavioral Therapy (CBT), and SBIRT.
  • Collaborate closely with primary care and psychiatric prescribers to provide consultation, care coordination, and integrated treatment planning.
  • Engage clients through walk-in visits, scheduled appointments, and warm handoffs to increase access and reduce barriers to care.
  • Advocate for clients to ensure their clinical, social, and systemic needs are addressed.
  • Partner with health navigators to identify and address social determinants of health impacting overall well-being.
  • Support and educate primary care team members in the integration and delivery of behavioral health services within a medical setting.
