About The Position

Trellis Health, a start-up subsidiary of MaineHealth, is looking for a passionate Behavioral Health Clinician to join their care team! The Behavioral Health Clinician - Ambulatory Care has strong clinical expertise and a passion for integrated, innovative care. As a key member of our interdisciplinary team, the Behavioral Health Clinician provides episodic, goal-oriented behavioral health services in close collaboration with medical and advanced practice providers. This role helps shape care plans, enhance the patient experience, and improve overall health outcomes through evidence-based, patient-centered support. Trellis Health is redefining primary care through a connected, compassionate, whole-person approach.

Requirements

  • Master's degree in behavioral health (LCSW, LCPC, or LMFT) from an accredited institution.
  • Current State of Maine Social Work license.
  • Experience providing clinical behavioral health services to diverse patient populations, including those with chronic medical issues.
  • Experience working in a practice that has integrated LCSW into a primary care workflow.
  • Proven ability to provide crisis intervention and de-escalation techniques.
  • Knowledge of professional social work principles, methodology, and ethics and of human psychosocial development within the family, community, and culture.
  • Knowledge of the use of therapeutic relationships to foster patient involvement.
  • Knowledge of health and social issues impacting diverse clients and their well-being.
  • Skill in client advocacy and case management.
  • Excellent communication skills, both written and verbal.
  • A high level of analytical skills to resolve complex psychosocial problems associated with illness.
  • Ability to interact effectively as members of interdisciplinary health care team.
  • Ability to identify and utilize community resources.
  • Ability to communicate appropriately with diverse patient populations.
  • Ability to maintain patient confidentiality.
