Summary: The Behavioral Health Clinician II delivers direct care, assessments, and therapeutic services to individuals experiencing acute mental health and substance use challenges. Working within a collaborative, team-based environment, the clinician provides ongoing care management and clinical support through comprehensive assessment, care planning, implementation, and continuous evaluation of each individual's needs.

Requirements

  • Master's degree in Social Work, Counseling or other human services related field.

Responsibilities

  • Conduct comprehensive psychosocial assessments and risk evaluations for patients identified as having safety concerns risk.
  • Develop individualized treatment plans in collaboration with the patient, family, and treatment team.
  • Facilitate individual, group, and family therapy sessions using appropriate clinical modalities.
  • Participate in daily multidisciplinary team meetings to coordinate care and support discharge planning.
  • Work collaboratively with psychiatrists, nurses, and support staff to ensure cohesive care delivery.
  • Provide crisis intervention and de-escalation support as needed.
  • Maintains a working knowledge of North Carolina state statutes related to the involuntary commitment process.
  • Document clinical interactions and progress notes in accordance with hospital policies and regulatory standards.
  • Assist patients and families in understanding treatment goals, discharge plans, and referrals to outpatient services.
  • Coordinates and facilitates safe discharge plans by connecting individuals to appropriate community resources and support services.
