About The Position

San Luis Valley Health in Alamosa, Colorado is seeking a highly motivated full-time Behavioral Health Clinician to provide behavioral health therapy services. This position will work closely with medical teams to support patient outcomes in preventative screenings, safety assessments, crisis intervention, and psychological first aid. The Clinician I will make referrals and connections with resources and collaborate with medical providers on planning and providing support to patients throughout the continuum of behavioral health services and care.

Requirements

  • Master's degree in counseling or social work-related field required.
  • Current BLS
  • Ability to receive feedback, learn from multiple professionals, and receive direction
  • Ability to communicate clearly and professionally with patients, providers, colleagues, and supervisors
  • Basic technology skills required

Nice To Haves

  • Trained in crisis and emergency intervention, preferred.
  • Licensed in mental health specialty, preferred.
  • Experience with screening, assessment and mental health counseling preferred
  • Experience in Substance Use Disorder and recovery preferred
  • Experience in crisis intervention, mental health first aid, and/or psychological first aid preferred.

Responsibilities

  • Provide increased access to behavioral health consultation, up to 50% of time, to Conejos County Hospital (CCH). This effort is provided through the Rural Healthcare Provider Expanded Access grant through the State of Colorado and American Rescue Plan Act (ARPA).
  • Work with a multidisciplinary team in multiple locations, including RMC and CCH hospital floors and Emergency Departments, Labor and Delivery, and various clinics, to address psychological and social needs of patients.
  • Proactive patient rounding in assigned locations, and attends daily multidisciplinary team rounds/ huddles.
  • Monitor Emergency Departments' status board to identify patients that may have any BH/SUD concerns.
  • Conduct chart scrubbing to identify patients in need in assigned departments.
  • Conduct proactive and as requested basic screenings of patients, including review of barriers to health self-management; depression; stress; and substance use.
  • Conduct screenings, brief interventions and referrals to treatment regarding ongoing substance use, crisis intervention, or behaviors that are in need of further care, placement review, or prevent readmissions.
  • Refer/connect patients to community resources.
  • Provide basic crisis intervention services to patients in need, and coordinate emergency response. This includes basic suicide screens, safety planning, and coordination with community partners.
  • Provide basic case management supports regarding health and mental health needs, functional abilities, readiness to change, caregiver stability, social supports, financial resources, environment and safety concerns, life care planning and self-management skills. This includes ensuring that patients discharged following mental health evaluations are contacted within 48 hours and care planning is addressed.
  • Help facilitate communication between patients and their providers, and coordinate communication among all of patient's multiple providers.
