About The Position

Requirements

  • Master's degree in Social Work (MSW).
  • California Licensed Clinical Social Worker (LCSW).
  • 2-3 years of relevant experience in a multidisciplinary healthcare setting.
  • Working knowledge of general theoretical and clinical models.
  • Ability to develop effective rapport and therapeutic alliances with patients.
  • Skill in writing clear and complete clinical notes and summaries.
  • Experience in documenting medical records in accordance with established policies.
  • Knowledge of LCSW ethics and scope of practice.
  • Ability to perform complete mental status exams and recognize changes in mental status.
  • Ability to establish and maintain effective relationships with professionals and support staff.
  • Knowledge of current community resources related to mental health and social services.

Responsibilities

  • Serve as a resource for providers, patients, and care coordinators.
  • Conduct urgent assessments and interventions for individual patients and their families.
  • Recruit for and coordinate group therapy sessions.
  • Advocate for patients and assist them in identifying coping skills.
  • Collaborate with community resources and managed care operations to meet healthcare needs.
  • Foster continuity of care and reduce unnecessary healthcare service utilization.
