About The Position
The Behavioral Health case manager addresses the needs of patients who are referred by CCHC providers, based on an assessment of Social Determinants of Health (SDoH). The BH Case Manager supports patients by directly linking them to community mental health and social service resources in order to achieve better health outcomes.
