The Behavioral Health Case Manager (BHCM) serves as a point of contact and advocate to members. They provide face-to-face, phone, virtual, and in-home services as needed by each member's journey to recovery. They develop, implement, and update person-centered individual service plans (promoting member participation) in a timely manner. Monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting the progress on the service plan as defined by policies and procedures. Ensures ongoing collaboration, including the communication of appropriate clinical information regarding progress of the person served with other individuals and/or entities with whom delivery and coordination of covered services is important (e.g., primary care providers, adult probation, other involved service providers). A successful Behavioral Health Case Manager works to ensure ongoing collaboration, communication, documentation, coordination of covered services, tracking progress of individuals treatment needs, and goals.

Requirements

  • Bachelors or master’s degree, no experience necessary or
  • Associate degree plus two (2) years behavioral health work experience or
  • High School Diploma or GED plus Peer Support Specialist certification plus three (3) years of behavioral health work experience or
  • High School Diploma or GED plus four (4) years of behavioral health work experience.
  • Must be able to pass a pre-employment drug screen and background check.
  • Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a “clean” or “acceptable” driving record.
  • At time of job offer, submission of 36 month (three year) driving record from DMV will be required.
  • Applicants must be able to pass a pre-employment drug screen.
