As a member of the Sunset team in support of a patient-centric approach to improve the health and well being of Sunset patients. Under clinical oversight by the Behavioral Health Director, will help patients achieve functional and/or disease goals through treatment planning and care follow-up. This position monitors results that have been articulated in a shared treatment plan between medical and behavioral health team. Work is performed in accordance with State of Arizona Behavioral Health Agency rules, regulations and standards, but requires the use of considerable independent judgment. Maintain the following core competencies: Interpersonal Communication: the ability to establish rapport and to communicate effectively with Sunset patients, their family members, care team members, and other providers. Screening and Assessment: Behavioral health screening to determine eligibility for admission; Assist in conducting and documenting Screening, Brief Intervention and Referral to Treatment (SBIRT) early intervention; Mental health assessment; Alcohol and/or drug assessment; Needs assessment, including Social Determinants of Health (SDOH) and, Conduct or arrange for more detailed assessments when indicated. Care Planning and Care Coordination through the following: Developing, implementing, and reviewing service plans for patients on assigned case load; Ensure access to an array of linked services in alignment with treatment plan; Coordinate and facilitate the exchange of information among patients and care team members, including case consultation with care team members; Complete appropriate referrals for treatment services within agency and all other outside agencies as needed; and, Consult with community agencies and families to maintain coordination in the treatment process. Identify high risk patient populations and conduct outreach to improve quality of care and patient outcomes, Maintain relationships with community partners to enhance the effectiveness of care networks, providing community education as needed. Patient Education: Be able to provide basic patient education around chronic medical conditions, mental health, substance use, and wellness Have a working knowledge of Motivational Interviewing Informatics: the ability to use information technology in support of project and program efforts as directed by executive leadership and Board. Utilize reports to track patient follow-up and clinical outcomes. Under the guidance of the BH Director will assist in a support capacity for Behavioral Health Technicians: Regarding allocation of duties; Training and re-training of new technicians and re-training of current staff; Review of documentation requirements; Support in additional areas when directed by the BH Director. Will ensure confidentiality for clients at all times. Comply with all training requirements per ADHS/AHCCCS . Complete other duties as assigned.
