About The Position

ESSENTIAL JOB RESPONSIBILITIES: Will provide Case Management Services to a caseload of approximately 25 Participants. Will apply the Housing First approach to serving people with housing and other service needs. Will submit routine reports and outcome data, per contractual requirements, and will utilize Dr. Cloud Will conduct necessary assessments, complete mandated legal documentation, and complete Individualized Service Plans (ISP) according to funding source and policy timelines. Will plan, schedule, and coordinate meetings with Participants, and facilitate route home visits as required by stated performance measures. Will research, access, and link to appropriate medical, psychiatric, education, community, and other social services/resources, as indicated by assessments and individual participant preferences. Will communicate and work collaboratively with other providers, leasing managers, and/or landlords to ensure appropriate service provision. Will meet with the supervisor for regularly scheduled meetings and as assigned by the supervisor. Will attend and actively participate in regularly scheduled staff meetings as assigned by the supervisor. Will attend and actively participate in required training, as assigned.

Requirements

  • Bachelor’s Degree in Social Service or Human Services related field.
  • Must have their own reliable transportation.
  • Willing to travel during the day to outlying parishes as required.
  • Able to multitask and meet deadlines.
  • Excellent written and verbal communication skills.
  • Proficiency in the use of Microsoft Office Software (i.e. Word, Excel) and computer literate.
  • Must be a team-player.
