About The Position

SUMMARY: A behavioral health case manager assesses, plans, implements, monitors care for individuals with mental health or substance use conditions. They coordinate services, advocate for clients, address barriers to care, and work to improve their overall well-being and independence. This often involves connecting clients with resources like community resources for basic needs, therapy, medication management, support groups.

Requirements

Educational degrees must be from a US Department of Education accredited school
AA or Bachelor’s degree in social work, psychology, or a related field plus three (3) or more years’ experience in behavioral health, substance use, or case management.
All individuals must demonstrate a commitment to the provision of culturally sensitive counseling services to American Indians.

Responsibilities

Provides on-going supportive and/or case management functions assisting other BH clinicians to equip their clients with needed internal and external resources and skill building.
Partner with designated external and internal interdisciplinary providers, programs, entities to address the needs, gaps in care, and recovery goals of complex and high risk community members.
Identify and remove barriers to procurement, delivery, participation in and success of services
Provides an orientation to services for individuals in need of services.
Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.
Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
Calmly and effectively address crisis situations as part of a team which may entail home visits
Assist community members and their families/caregivers with connections to appropriate psychiatric, medical, and psychosocial referrals and services.
Advocacy with community partners to represent clients' needs and rights, ensuring they receive appropriate care and support, and connecting them with necessary resources.
Provide 1:1 behavior intervention for clients based on client’s treatment plan.
Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements.
Regularly communicate/consult with Client’s counselors, Clients’ treatment team, and/or Manager and Director regarding client’s behavior, progress and treatment goals.
Participate in care team or child/family team meetings.
Attend staff trainings and in-services.
Sensitive and responsive to the cultural differences among the agency’s client population.
Active participant in quality improvement projects.
Utilize strategies and interventions pertinent to individual clients, including, but not limited to, culturally relevant practices, traditions and preferences and a strengths-based harm reduction approach.
Refer clients to additional treatment, services or support, as indicated. Coordinate services with referring agencies as needed.
Adhere to UIHS Communication Norms and Guiding Principles, accreditation and compliance standards/guidelines.
Abide by all legal and ethical standards of the profession including as dictated by state, federal, accrediting and licensing body guidelines.
Performs other training and job related activities requested by the Behavioral Health Director and Manager.
Maintains a professional organized clean working environment by following organizational policies, guidelines, and safety standards.
Willing to learn more about the American Indian culture and attend at least one Cultural Presentation/Training per year.
Willing to work with American Indian/Alaska Native people and demonstrate cultural awareness and humility.
Willing to deliver services in a culturally responsive manner.
Adheres to accreditation and compliance standards/guideline.
Other duties as assigned.
In collaboration with a licensed counselor, may provide specialized counseling groups or classes, on topics such as parenting, depression, grief, anger management, self-esteem, youth groups or other topics within the scope of skill and training.
Provide systems-oriented interventions to develop parenting skills for client’s family, foster family or support system.
Participates in reciprocal education with other providers to support integration of peer support and recovery principles within provider teams.
Provide intensive case management services to higher need community members with SMI, dual diagnosis, and other complex presentations.
