About The Position

The Behavioral Health Care Manager is a licensed clinical social worker or marriage and family therapist responsible for providing ongoing behavioral health support to patients with depression and anxiety enrolled in Welby Health’s integrated care programs. In this position, you will deliver brief, evidence-based interventions, monitor clinical progress using standardized measurement tools, and coordinate care in collaboration with primary care providers and consulting psychiatrists. As a Behavioral Health Care Manager at Welby Health, you will serve as the primary point of contact for patients between primary care visits, ensuring continuity of care through structured follow-up and timely interventions. Furthermore, you will collaborate closely with psychiatrists through regular case reviews to guide treatment planning, implement evidence-based recommendations, and escalate care appropriately to support optimal patient outcomes.

Requirements

  • Active and unrestricted license as a Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT)
  • Holds valid licensure in one or more states with ability to provide care across multiple jurisdictions
  • Experience working with adult patients with depression and anxiety disorders
  • Exceptional written and verbal communication skills
  • Ability to work independently in a remote, technology-enabled environment
  • Strong organizational skills with attention to detail and follow-through

  • Experience in integrated care, collaborative care models, or primary care behavioral health settings
  • Familiarity with measurement-based care and outcomes tracking
  • Experience working with complex or high-utilization patient populations

Responsibilities

  • Conduct initial behavioral health assessments for patients enrolled in the program
  • Provide regular follow-up through brief, structured interventions focused on symptom management, behavioral activation, adherence support, and care navigation
  • Administer and track standardized assessment tools to monitor symptom trends and identify patients requiring treatment adjustments
  • Prepare and present cases during weekly psychiatric consultation, including patients who are new to the program, not responding to treatment, or clinically complex
  • Implement psychiatric recommendations in collaboration with primary care providers and escalate safety concerns according to established protocols
  • Coordinate care with primary care teams, existing behavioral health resources, and community services to ensure seamless transitions and continuity across care settings
  • Document patient interactions accurately and completely in accordance with internal protocols and regulatory standards
  • Maintain consistent patient engagement and adhere to established clinical workflows and program protocols
