About The Position

The Behavioral Health Care Coordinator consults with the patients behavioral health provider and primary care provider regarding treatment options. The Care Coordinator coordinates care with other clinics and agencies that care for the patient. The Care Coordinator educates the patient about physical and mental health issues and gathers social history information.

Requirements

  • High School Diploma/GED

  • Experience with depression and depression treatment plan
  • Working with medically ill and/or older adults
  • Prior exposure to brief, structured counseling techniques (i.e. cognitive behavioral therapy (CBT), psychological skills training (PST), or SBIRT)

Responsibilities

  • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
  • Will manage the Clinic Event Manager (disease registry) for all patients to ensure compliance with all preventative and chronic care tests, treatments, etc. to ensure highest quality of care.
  • Will manage the panels for Behavioral Health.
  • Reviews program requirements and social history with patients.
  • Identifies patient service requirements by reinforcing personal rapport with potential and actual patients and other persons in a position to understand service requirements.
  • Promotes patient's independence by establishing patient care goals; teaching and advising patient, friends, and family and reinforcing their understanding of disease, medications, and self-care skills.
  • Provides information to patients and health care team by answering questions and requests.
  • Maintains a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
  • Contributes to team effort by accomplishing related results as needed.
  • Consults with patient, psychotherapist, and primary care provider about treatment options and preferences; coordinates initiation of treatment plan.
  • Monitors patient closely (in-person, by email, or by phone) for changes in severity of symptoms and medication side effects; educates patients about medications and medication side effects, as needed; encourages treatment adherence.
  • Uses behavioral activation, motivational interviewing, and problem solving techniques with patients as an adjunct to other treatments.
  • Coordinates and facilitates communication between patient, primary care physician, psychotherapist, and consulting psychiatrist; provides recommendations for change in treatment plan according to evidence-based algorithm and expert supervision; supports implementation of new plan.
  • Documents all encounters according to organizational policies and procedures; monitors outcome measurements.
  • Facilitates treatment referrals, as needed.
  • Completes relapse prevention plan with patients who are in remission.
