About The Position

The Behavioral Health Care Coordinator functions as a member of the Program of All-Inclusive Care for the Elderly (PACE) collaborative care team that includes the patient, the interdisciplinary team (IDT), and other medical staff. This is a non-clinical position that does not provide therapy or counseling. This position is responsible for providing integrated behavioral health care, including case management, within a PACE care setting, which includes planning, organizing, and coordinating behavioral health services in conjunction with other members of the patient’s care team and identifying external providers who impact patient care. This includes transitions of care and all associated administrative duties.

Requirements

  • Bachelor’s degree in a mental health field (social work, psychology, counseling, or related field).
  • One (1) year experience working with individuals with co-occurring mental health and/or substance use disorders; individuals with a disability; individuals of a vulnerable population (elderly, mothers, children) and/or other high risk groups preferred.
  • Suicide assessment, treatment, and management training required by WA State Department of Health.
  • Ability to travel within the Seattle area as needed.
  • Valid Driver's License and Proof of Insurance required, if applicable.
