About The Position

Learn more about us at www.lchcs.com Hours: Monday – Friday, 8:00 am – 5:00 pm Qualifications: Must be high school graduate. Training and experience preferred but not required. Job duties include but are not limited to: Documents blood pressure, temperature, pulse, respirations, height and weight during work up in electronic medical record. Administers nursing care to patients according to their physical and emotional needs and in accordance with the medical plan of care. Sets up, dismantles and cleans exam room for Provider. Performs or assists with procedures as directed by the Director of Nursing and Provider staff. Ensures safety of patients. Participates in the patient/family education process as directed by Provider. Assumes responsibility for ensuring continued professional and/or technical growth. Observes records and reports all significant information, signs and symptoms to the Director of Nursing and/or Provider. Assists patients and prepares equipment for Provider examinations. Maintain strict confidentiality of all protected health information. Maintain timely and accurate data entry and reporting. Communicates directly and frequently with the Director of Nursing and Provider; reports progress and concerns. Assists with the Reach Out and Read Program. Attend trainings as specified throughout the year. Complete the pre-visit checklist. Meet with patients to obtain history and complete pre-visit Reach out to patients by phone and verify appointments Reschedule appointments Verify patient demographics Keep family members contact information (phone tree) Assist provider with forms from current visit and for the follow up Make copies of handouts for the patients Make internal referrals Maintain the waiting list of patients and coordinate schedules Medication refills Attend all MH meetings May be asked to occasionally work with Dr. Abe or Wren Track 90-day progress summary due dates for patients Perform any other duties as assigned by supervisor. Salary scales are a range of pay and offers are made to applicants based upon the requirements of the particular position. Applicants with “previous like skills” may not be given the same weight and consideration as “previous specific same skills”: i.e., For a Medical Receptionist position, “Previous clerical skills” may not be preferred over “Previous Medical Clerical Skills”.
