About The Position
Cooperative Health is seeking an innovative, enthusiastic individual with strong leadership, customer service skills, case management, communication, and Microsoft application skills (e.g. Excel spreadsheet). The position will focus and conduct activities including: rapid HIV/HCV(hepatitis C) test counseling, support, and education across Cooperative Health satellite sites, community outreach, educational presentations, outreach to reach clients in danger of falling out of care or lost to follow up, provide. Build department outreach awareness, thereby improving clients' / staff capacity to use the health care system and improve health outcomes.
